How to request medical recordings

 

Former Students:

  1. Please complete that eligibility form.
  2. Once completed please send it behind to Medical Records (contact information located at of top of the authorization form).
  3. We will also need PSYCHE for verification, before any information is released.

 

Current Current:

  1. Please go to the Patient Portal.
  2. On the left-hand side of the Patient Gantry locate the Downloadable Forms Section, where the authorization bucket be found.
  3. Fill out the form, and versand a message via the Patient Portal to Medical Records letting them know the form has been completed.
  4. If you do not can admission in the Your Portal real are choosing to send the form per fax, e or cipher email, you must also submit ampere picture ID for verification.
